MyAppointmentPal Terms of Service
Effective date: October 20, 2026
1. Agreement to These Terms
These Terms of Service ("Terms") are an agreement between you and MyAppointmentPal, LLC ("MyAppointmentPal," "we," "us," or "our"), a Florida limited liability company located at 5348 Watervista Dr, Orlando, FL 32821. They govern your use of myappointmentpal.com (the "Website") and the companion escort and transportation services we provide (the "Services").
Before completing a purchase for a Service, you will be provided an opportunity to review these Terms and must affirmatively acknowledge and accept them through the Website or other authorized booking platform. Acceptance may be provided electronically, including by checking an acceptance box or using another affirmative electronic acceptance method presented at booking. The Terms and our Privacy Policy will be made accessible to you as part of the booking process, and your acceptance may be retained electronically as a record of your agreement. By booking a Service, checking the box at booking, or using the Website, you agree to these Terms and our Privacy Policy. If you book on behalf of someone else (such as a parent or a patient), you confirm you have authority to do so, and you agree to these Terms both for yourself and on behalf of that person (the "Patient"). By accepting these Terms on behalf of a Patient, you represent that you have the authority to accept these Terms on the Patient's behalf. Your acceptance also acknowledges the risks associated with the Services, including transportation and post-procedure accompaniment, and the assumption of risk, release, and limitation of liability provisions contained in these Terms. If you do not agree, do not use the Website or Services.
2. Our Services
MyAppointmentPal provides non-medical companion escort and transportation for people attending medical appointments and procedures. We offer three service types:
Full Companion Ride: Includes pickup from the Patient's home, transportation to and from the scheduled procedure, a companion who waits for the entire procedure, and optional prescription pickup on the return trip (prescription costs paid by the Patient). All companions are background-checked, CPR-certified, and provide non-medical accompaniment only.
Discharge Ride: A MyAppointmentPal companion meets the Patient at discharge, goes over the discharge instructions with the Patient, and gets the Patient safely home. All companions are background-checked, CPR-certified, and provide non-medical accompaniment only.
Recovery Support: A companion booked by the hour (1-hour minimum) to stay with the Patient after the procedure. The companion helps the Patient settle in at home, reviews or relays the discharge instructions provided by the facility to the Patient, picks up prescriptions (prescription costs paid by the Patient), prepares a light meal, and runs small errands. All companions are background-checked, CPR-certified, and provide only non-medical assistance.
We are not a healthcare provider. Our companions do not provide medical care, nursing care, or medical advice, even if they hold a nursing or CNA credential. Companions will not:
Administer, prepare, or manage medications
Perform medical tasks, wound care, or physical therapy
Lift, carry, or perform transfers of the Patient
Make medical or legal decisions for the Patient or sign consent forms for treatment
Give medical opinions about the Patient's condition
Transportation Limitations. The Services are limited to transportation and non-medical companionship for Patients who can safely enter and exit a standard passenger vehicle with minimal assistance. We do not provide wheelchair transportation, stretcher transportation, ambulance transportation, medical transportation, or transportation requiring lifting, carrying, transferring, or other physical assistance beyond what is expressly described in these Terms.
No Medical Assessment or Clinical Judgment. MyAppointmentPal does not evaluate, diagnose, monitor, or determine whether a Patient is medically fit to undergo a procedure, leave a medical facility, travel in a motor vehicle, or remain alone or with a companion following a procedure. The Patient and the Patient's healthcare providers are solely responsible for determining whether the Patient is medically appropriate for the Service.
A companion may relay discharge instructions as given by the facility, but the facility remains responsible for the Patient's care and instructions. The facility decides whether to release the Patient to our companion; we are not responsible if it declines.
Independent contractors. Companions and drivers providing Services are independent contractors and are not employees of MyAppointmentPal. Drivers use their own vehicles and are responsible for maintaining the insurance and licenses required for their transportation services. MyAppointmentPal does not provide medical care through its independent contractors. The independent contractor relationship does not expand the scope of Services described in these Terms or make any contractor responsible for services that MyAppointmentPal does not offer.
Emergencies. If the Patient appears to need urgent medical help at any point, the companion will call 911 and contact the emergency contact on file. Companions are not trained or permitted to provide emergency medical treatment beyond CPR. MyAppointmentPal and its companions do not guarantee that emergency assistance will be available within any particular period, and the Patient remains responsible for providing accurate emergency contact information and promptly following instructions from emergency personnel and healthcare providers.
Patient Safety and Right to Decline or End Service. MyAppointmentPal may decline, suspend, or terminate a Service at any time if, in its reasonable judgment, the Patient's physical condition, mobility limitations, behavior, medical needs, or other circumstances make the Service unsafe or outside the scope of the Services described in these Terms. This includes circumstances in which the Patient requires lifting, carrying, transferring, wheelchair or stretcher transportation, medical supervision, medication assistance, or other care that MyAppointmentPal is not authorized or equipped to provide. If a Service is declined or terminated for safety reasons before transportation begins, MyAppointmentPal will determine any applicable refund or credit in accordance with its cancellation policy.
3. Eligibility, Booking, and Payment
Eligibility. The person booking must be at least 18. The Patient must be able to safely enter and exit a standard passenger vehicle with minimal assistance and must not require lifting, carrying, transferring, wheelchair transportation, stretcher transportation, or medical assistance. Patients under 18 are not served / must be accompanied by a parent or legal guardian.
Accurate information. You agree to give us accurate, complete information, including pickup and drop-off addresses, appointment time, facility name, mobility needs, and an emergency contact. You must notify us before the Service of any known condition, limitation, or circumstance that could affect the Patient's ability to safely receive the Service, including any anticipated need for physical assistance, mobility equipment, special transportation, or post-procedure supervision. We may decline, adjust, or end a Service if the information is inaccurate or the Patient's needs exceed what our companions can safely provide.
Service area. We currently serve Orlando, Melbourne, Tampa, Sarasota, West Palm Beach, Fort Lauderdale, and Jacksonville. Availability depends on companion schedules.
Payment. Payment is due at booking. Prices are shown on the Website or in your booking confirmation. We accept major credit and debit cards, including HSA/FSA cards where your plan allows; you are responsible for confirming eligibility with your plan administrator. Our companions and drivers do not accept payment of any kind, including cash; all payments are made directly to MyAppointmentPal, LLC.
Extended wait time. Each booking includes a 30-minute grace period beyond the booked time. After the grace period, additional time is billed at $50 per hour in full-hour increments (any part of an hour is billed as a full hour), charged to the payment method on file. Delays caused by the medical facility, procedure, discharge process, Patient, or any other circumstance outside MyAppointmentPal's reasonable control may result in additional waiting charges.
Conduct. We may end a Service without refund if the Patient or anyone with them is abusive, threatening, intoxicated in a way that creates a safety risk, or asks the companion to do something unsafe or outside these Terms.
4. Cancellation and Rescheduling Policy
Deadline: Changes and cancellations must be made by 9:00 PM Eastern the day before your service. To cancel or reschedule, call or text 407-861-2446 or email booking@myappointmentpal.com. The time we receive your message is what counts.
Cancel before the deadline: Choose a refund of the amount paid, less a $75 cancellation fee, or a full account credit toward a future booking. Account credits must be used for a service date within 12 months of the cancellation date.
Recovery Support bookings: The cancellation fee applies to Full Companion Ride and Discharge Ride bookings only. Recovery Support bookings canceled before the deadline receive a full refund.
Cancel after the deadline or no-show: Bookings are non-refundable after 9:00 PM Eastern the day before service. This includes bookings made after that time.
Your procedure is canceled or rescheduled by the facility: The Patient is responsible for promptly notifying MyAppointmentPal of any change, cancellation, or rescheduling communicated by the medical facility. With confirmation from the facility, we'll issue a full account credit, subject to the same 12-month limit. MyAppointmentPal is not responsible for determining whether a procedure has been canceled or rescheduled by the medical facility.
Rescheduling: Change your date or time free of charge through 9:00 PM Eastern the day before service, subject to companion availability. The new date follows the same policy.
If we cancel: You receive a full refund.
Refunds go back to your original payment method, including HSA/FSA cards, within 5–10 business days.
A no-show includes a Patient who is not at the pickup location within 15 minutes after the scheduled pickup time and cannot be reached after MyAppointmentPal makes a reasonable attempt to contact the Patient using the contact information provided at booking. A no-show also includes a Service the facility declines to proceed with because the Patient did not follow its pre-procedure instructions (for example, fasting). The Patient is responsible for following all pre-procedure instructions provided by the medical facility. Account credits have no cash value and are not transferable.
5. Health Information and Privacy
To provide the Services, you may share limited health information, such as the type of procedure, sedation, and mobility needs. MyAppointmentPal is not a healthcare provider, health plan, or HIPAA "covered entity." Even so, we keep the health information you share confidential, use it only to provide and coordinate your Service, and share it only with the companion assigned to you, the facility as needed, or your designated emergency contact, or as required by law. You are responsible for providing accurate information concerning the Patient that is reasonably necessary for us to safely coordinate the Services. We are not responsible for consequences resulting from materially inaccurate, incomplete, or omitted information provided by you or the Patient.
Your use of the Website and Services is also governed by our Privacy Policy, which explains how we collect, use, and protect your information.
6. SMS Terms
By providing your mobile number and opting in, you agree to receive recurring text messages from MyAppointmentPal about your bookings and account, such as confirmations, reminders, and companion updates. Message frequency varies based on service activity. Message and data rates may apply. Consent to receive texts is not a condition of purchase.
Help: Reply HELP for help, or contact booking@myappointmentpal.com.
Stop: Reply STOP to cancel at any time. We will send one confirmation text, and no further messages will be sent unless you opt in again.
Carriers are not liable for delayed or undelivered messages.
Privacy: Mobile information, including text opt-in data and consent, will not be shared with third parties or affiliates for marketing or promotional purposes. See our Privacy Policy.
7. Assumption of Risk and Release
Risks you understand. The Services involve riding in a motor vehicle and walking to, within, and from medical facilities, often after sedation or anesthesia. These activities carry inherent risks, including traffic accidents, falls, delays, and reactions to medical procedures. Additional risks may include dizziness, weakness, impaired balance, confusion, delayed effects of sedation or anesthesia, difficulty or inability to safely enter or exit a vehicle, unexpected changes in the Patient's condition, delays in discharge, traffic conditions, and other vehicle-related risks or risks inherent in providing the Services.
What we are not responsible for. We are not responsible for: (a) the medical care, decisions, complications, or outcomes of any procedure; (b) the acts or decisions of the facility or its staff, including declining to release the Patient; (c) appointments missed or delayed because of traffic, weather, road conditions, or facility scheduling; or (d) communication between you and your healthcare provider.
Release of Claims. To the fullest extent permitted by Florida law, you, on behalf of yourself and, to the extent legally authorized, the Patient, release and agree not to hold MyAppointmentPal, LLC, its members, officers, agents, employees, independent contractors, companions, and drivers responsible for claims, demands, losses, or damages arising out of or relating to the inherent risks of the Services, including transportation, entering or exiting a vehicle, walking to or from a medical facility, post-procedure weakness, dizziness, confusion, sedation or anesthesia-related effects, falls, traffic conditions, delays, or other risks inherent in the Services, including claims based on ordinary negligence to the extent such claims may lawfully be released under Florida law.
This release does not apply to claims arising from gross negligence, willful misconduct, or any liability that cannot lawfully be released or limited under applicable law.
Patient and Customer Responsibility. The Patient and the person booking the Service are responsible for following all instructions provided by the Patient's healthcare providers and medical facility, including instructions concerning discharge, transportation, post-procedure care, medications, and supervision. MyAppointmentPal does not determine whether the Patient is ready for discharge or transportation. The Patient must inform MyAppointmentPal of any condition or limitation that could affect the safe provision of the Services.
Personal belongings. Please keep valuables with the Patient. We are not responsible for lost or damaged personal items unless caused by our gross negligence or willful misconduct.
8. Indemnification
You agree to indemnify and hold harmless MyAppointmentPal, its members, officers, and independent contractors from claims, losses, and reasonable attorneys' fees arising from: (a) false, incomplete, or misleading information you provide, including about the Patient's health or mobility, including information concerning the Patient's mobility, physical condition, transportation requirements, or other circumstances relevant to the safe provision of the Services; (b) your or the Patient's violation of these Terms; or (c) your or the Patient's intentional misconduct or damage to a companion's vehicle or property.
9. Disclaimer and Limitation of Liability
The Website and Services are provided "as is" and "as available." To the extent permitted by law, we disclaim all implied warranties, including merchantability and fitness for a particular purpose.
To the fullest extent permitted by law, we are not liable for any indirect, incidental, special, consequential, or punitive damages. Our total liability for any claim relating to the Website or Services will not exceed the greater of the amount you paid for the Service giving rise to the claim or $10,000. These limits do not apply to liability that cannot be limited under Florida law.
Transportation Disclaimer. MyAppointmentPal provides non-medical transportation and companion services only. The Services are not ambulance, emergency medical transportation, wheelchair transportation, stretcher transportation, or medically supervised transportation. We do not guarantee that transportation will be available in every circumstance or that a Patient will arrive at a medical facility or residence by any particular time.
10. Dispute Resolution and Arbitration
Please read this section carefully. It requires arbitration of certain disputes and limits the manner in which claims may be brought. By affirmatively accepting these Terms before purchasing a Service, you acknowledge that you have had an opportunity to review this section and agree to its terms.
Informal resolution first. Before filing a claim, contact us at info@myappointmentpal.com and give us 30 days to try to resolve it.
Arbitration. Any dispute that is not resolved will be settled by binding arbitration administered by the American Arbitration Association under its Consumer Arbitration Rules, held in Orange County, Florida, or by video conference. The arbitration will be conducted on an individual basis and not as a class, collective, or representative proceeding. Either party may instead bring an individual claim in small claims court if it qualifies.
No class actions. Claims may be brought only on an individual basis, not as a plaintiff or class member in any class or representative proceeding.
Opt-out. You may opt out of this arbitration section by emailing info@myappointmentpal.com within 30 days of first accepting these Terms. Your opt-out request must clearly state that you are opting out of the arbitration provision and include sufficient information for us to identify the booking or account associated with your acceptance of these Terms.
11. Website Use
We grant you a limited, non-exclusive, non-transferable license to use the Website to learn about and book Services, for yourself, for someone you are assisting, or on behalf of a medical practice or facility referring a Patient. You agree not to interfere with or disrupt the Website or use it in violation of any law. All content on the Website, including text, graphics, logos, and images, belongs to MyAppointmentPal and may not be used without our permission.
12. General Terms
Governing law: These Terms are governed by the laws of the State of Florida, without regard to conflict-of-law principles.
Changes: We may update these Terms by posting a new version with a new effective date. Changes apply to bookings made after posting and do not apply to disputes already pending.
Force majeure: We are not liable for delays or cancellations caused by events beyond our reasonable control, including hurricanes, severe weather, road closures, and public emergencies.
Severability: If any part of these Terms is found unenforceable, the rest remains in effect.
No Medical Provider Relationship. Nothing in these Terms creates a physician-patient, nurse-patient, healthcare provider-patient, fiduciary, or other clinical relationship between MyAppointmentPal or its companions and the Patient. MyAppointmentPal does not act as the Patient's healthcare provider, medical decision-maker, caregiver, or representative in connection with medical treatment.
Facility Requirements. The Patient is responsible for confirming with the applicable medical facility whether the facility permits discharge to a MyAppointmentPal companion and whether any additional responsible-adult, escort, transportation, or post-procedure requirements apply. MyAppointmentPal does not guarantee that any particular facility will accept a companion or permit the Patient to leave with a MyAppointmentPal representative.
Entire agreement: These Terms, your booking confirmation, and our Privacy Policy are the entire agreement between you and us about the Services.
Assignment: You may not transfer your rights under these Terms. We may assign them in connection with a sale or reorganization of our business.
